If a fee is required for permission to reprint, please provide your taxpayer identification number. Federal regulations require that this number be recorded with payment. If you do not control the rights requested, please advise me as to whom I should write.

Since the book is already in production, I would appreciate hearing from you as soon as possible about your permission, its terms, and the credit line that you wish me to use.
